چکیده

Cardiac tamponade resulting from hemopericardium after a thoracic trauma is a relatively common occurrence. We report on 2 cases of extrapericardial cardiac tamponade, a condition that is definitely less common although potentially life threatening. Both patients underwent sternotomic cardiac decompression and were transferred to ICU: the first patient died of neurological damage, the second patient was discharged on day six after surgery. The aim of the present report is to focus on the hemomediastinum, and in particular to stress the fact that a chest trauma apparently stabilized can suddenly lead to an extrapericardial tamponade.

Hiatal hernia causing extrapericardial tamponade after coronary bypass surgery.

Cardiac tamponade is defined as compression of the heart due to accumulation of fluid in the pericardial sac, leading to raised pericardial pressures with haemodynamic compromise. We describe the case of a 76-year old female patient who underwent a routine off-pump coronary artery bypass graft operation and within 48 h developed classic signs of cardiac tamponade. The perioperative echocardiogr...

Retrosternal Hemomediastinum Caused by Internal Mammary Artery Injury: A Case Report and Review of the Literature

We describe a 35-year-old multi-trauma patient who had a high-grade liver laceration with extensive retrosternal hemomediastinum. Extrapericardial tamponade secondary to the fracture of the sternum was followed by the instantaneous surgical evacuation of the hematoma. Computed tomography (CT) angiography helped diagnose this uncommon entity of blunt trauma-induced internal mammary artery bleedi...

Closure of pericardium after open heart surgery. A way to prevent postoperative cardiac tamponade.

Between July 1968 and December 1975, 821 patients underwent open heart operations. In 596 cases the pericardium was left open and in 225 the pericardium was closed. Forty-one patients in the open pericardium group required reoperation and 23 of these had tamponade. Four patients in the closed pericardium group had reoperation but there was not a single case of tamponade. In most cases that requ...
